Correction, September 2, 2026. An earlier version of this piece described enrolling a veterinary practice in the state spay/neuter program as “a form signed by the veterinarians in the practice, a W-9, and an address in Pike Road.” That left out a requirement. Everything in animal welfare is either preventing the next intake or managing the last one. Both matter. Only one is funded.

Here is the part of that sentence that does not apply cleanly to Alabama, and it is worth sitting with. Alabama does fund the upstream layer. It has since 2009. There is a specialty license plate, and when somebody buys or renews one, the state takes $8.75, the Alabama Veterinary Medical Foundation takes $6.19 for administration, and $35.06 goes to paying veterinarians for spay/neuter surgeries on the pets of Medicaid families. There is a tax return check-off too.

That is not a gimmick. That is a functioning, self-sustaining prevention fund, built by a veterinary association and paid for voluntarily by people who wanted a nicer tag. Since 2009 it has put $2,495,848 into sterilization and paid for 42,677 surgeries. The reimbursement goes to the veterinarian on top of what the client pays, so a rural practice is not being asked to donate labor: $100 for a dog spay, $50 for a dog neuter, $80 for a cat spay, $35 for a cat neuter. Roughly seven of every eight dollars that reaches the Foundation goes to surgery rather than overhead.

If you want to know whether Alabama believes prevention is worth public money, that question is already answered. It answered yes in 2007, before most of the field was having this conversation at all.

So run it through the lens. Is it prevention? Yes, unambiguously. It is one of the cleanest upstream mechanisms in the country: it pays for the surgery that stops the litter, it reaches the households least able to pay, and it costs the general fund nothing. Does it work? For the people it reaches, yes. Forty-two thousand surgeries is forty-two thousand animals that never produced anything.

The problem is the third question. What works about it, and what does not.

Divide 42,677 surgeries by the seventeen years the program has been running and you get about 2,510 a year. Alabama has 5,157,699 people. That is 0.49 surgeries per thousand residents per year.

The working benchmark in this field, and I want to be careful about how I present it, comes from practitioners rather than from a peer-reviewed study. Beverly Ashton, who has run this work in Athens and Limestone County for years and is opening her own low-cost clinic this fall, puts it this way: at three to four surgeries per thousand residents per year a pet population keeps growing, five to six holds it flat, and seven to eight starts bringing it down. Hold seven or eight for about five years and shelter intake falls close to half. Treat that as field-derived operational guidance, not as a law of nature.

Against that benchmark, the state program is running at about six percent of the dose.

That is not a criticism of the Foundation. They are doing what the revenue allows, and the revenue is capped by design. A specialty plate is $50 and the fund only grows when more people buy plates. Nobody at the ALVMF decided to operate at six percent. The legislature decided how big the pipe would be, in 2007, and then stopped thinking about it.

This is a coalition argument. Hand it to one person in your network who has not seen the math. That is how the shift starts.

Now the geography, which is the part that surprised me.

The Foundation publishes its list of participating clinics, updated April 15 of this year. Thirty-three veterinary hospitals in the entire state. Those thirty-three sit in nineteen counties. Alabama has sixty-seven counties. So forty-eight Alabama counties contain no clinic that participates in the state spay/neuter program.

If you live in one of those forty-eight counties and you are on Medicaid and you want the help the state is offering, the answer is that you drive. In a county where the nearest open veterinarian is already forty minutes away, that is not a program. That is a program you have heard about.

I looked at the seven counties Animal-Angels Foundation serves. Jefferson has five participating clinics. Walker has one. Blount, Shelby, St. Clair, Bibb, and Chilton have none. Five of seven.

Blount County is the one worth spending a paragraph on, because it corrects a story the field tells about rural counties. The easy version is that rural counties do not fund animal welfare. Blount does. There is a county animal services department. There is the Animal Adoption Center of Blount County in Cleveland. Oneonta runs its own animal control. Blount County is paying, right now, for the downstream layer, at full retail price.

What Blount does not have is a way in to the upstream layer. Four solo veterinary practices serve roughly 60,000 people across 650 square miles, and not one of them is enrolled in the state program. In the first quarter of this year, 479 animals entered that county shelter, and 54 percent of them were under six months old. That is not a shelter management problem. That is a litter pipeline, and it is running at full speed in a county that already pays for the consequences and has no funded mechanism to touch the cause.

The contrast that makes this concrete is a hundred miles north, and it needs to be told carefully, because Huntsville’s story is not only a spay/neuter story. Huntsville Animal Services took in 10,248 dogs and cats in 2009. By 2014 that was 5,899. A 42 percent drop in five years, from the city’s own reporting forms.

Then it stopped falling. 6,036 in 2023. 5,726 in 2024. Raw intake in Huntsville has been broadly flat for a decade, against a city that has grown considerably over the same period, which means the per-resident rate has kept improving even though the headline number has not. Anyone who tells you Huntsville cut its intake in half is describing 2009 through 2014, not the last ten years.

Huntsville also did several things at once, and the honest version credits that. The city funded sterilization. It also went through a documented shelter reform, driven by advocates who spent years pushing for it, and the definitive account of that part is Aubrie Kavanaugh’s book Not Rocket Science. The save rate transformation is her story to tell, and she tells it well. What I am pointing at is narrower and it survives her account intact: the decline came in a city that chose to put its own money into the front end, in a state where the only standing mechanism reaches under half a surgery per thousand residents and skips forty-eight counties entirely.

There is one more thing in the Huntsville reporting that almost nobody publishes, and it is the number I would put in front of a county commission. Their intake forms break every animal into adult or up to five months old. In 2023, 29.5 percent of Huntsville’s intake was under five months. In 2024, 29.0 percent. In April of this year, 31 percent. Steady, year after year, right around three in ten.

In Blount County, in the first quarter of this year, 54 percent of intake was under six months old. The cutoffs are not identical and neither are the periods, so treat that as a comparison of shape rather than a matched statistic. But the shape is the whole argument. In a place with a funded front end, roughly three in ten animals arriving at the shelter are puppies and kittens. In a place without one, it is better than half. That is the litter pipeline, visible in the intake sheet, and it is the thing spay/neuter money actually touches.

Under half a surgery per thousand residents is what the state mechanism delivers. That gap between a city that funded its own front end and a state that built one and never sized it is not about which community cares more. It is about which one had a funded delivery system.

So where does the license plate program belong in the ecosystem? It is a load-bearing idea running at pilot scale. It proves the model works in Alabama, with Alabama veterinarians, under Alabama rules, and it has seventeen years of receipts. It is also structurally incapable of reaching the dose, because its revenue is tied to how many people buy a specialty tag and its delivery is tied to how many practices happen to have enrolled.

That is worth being precise about, because the honest version of this argument is not that Alabama has done nothing. Alabama built the right thing and then never sized it. The debate in front of the legislature is not whether to start funding prevention. It is why we stopped at six percent, and whether the state is willing to fund the other ninety-four.

There are mechanisms that would move it. Five states now fund spay/neuter through pet food registration fees, which are already mandatory almost everywhere and currently produce nothing for animals. West Virginia, with a third of Alabama’s population, is delivering at a per-capita rate several times what Alabama’s plate program manages. Texas stood up a $13 million statewide spay/neuter pilot in 2026. None of that is theoretical, and none of it required a new tax on anyone.

And there is a smaller fix that does not require the legislature, though it is not as small as I first wrote it. Forty-eight counties have no participating clinic. A practice enrolls by submitting a hospital enrollment form signed by every veterinarian in the practice, a W-9, and an address in Pike Road. The part I left out is requirement number one: every veterinarian working at that hospital has to be an ALVMA member or have applied for membership, and new hires have 90 days to apply or the hospital loses participation. ALVMA dues run $220 a year for a veterinarian more than four years out of school, $110 for those three to four years out, free for students and new graduates.

So a solo rural practice pays $220 before it can bill the state for a single surgery, and a three-vet clinic where nobody is currently a member pays $660. It still pencils. Three dog spays at the $100 reimbursement clears the dues for a solo practice, and most participating clinics do far more than three. But it is a cost, not a phone call, and I should not have described it as one.

It is also worth naming what that structure does, without treating it as a villain. ALVMA has real administrative reasons to want the practices it vouches for inside its membership. The effect, though, is a dues wall standing between public money and the practices best positioned to spend it, and that wall is highest exactly where the need is greatest, at the one-veterinarian practice in the county with no participating clinic. If Alabama builds a larger prevention fund, and I think it should, that is an argument for housing it somewhere other than a membership organization.
